Welcome Home Program
This program utilizes a certified aide for 8 hours to assist patients being discharged from hospitals, nursing homes or rehabilitation centers . This includes transportation and any of the duties listed on the "101 Ways." This will insure a smooth transition from discharge to going home. It can also be used for out-patient surgery, eye surgery, dental visits and dental surgery.
